At On Deck Designs, we believe every player deserves to see themselves as the star they are — immortalized in a premium, professionally designed baseball card that they'll keep forever. But before we bring that vision to life, we need your permission.

Why We Require Consent

When you place an order with On Deck Designs, you are submitting photos and personal information (including your player's name, team, and likeness) to be used in the creation of custom baseball card designs. This consent form ensures that:

• You are the parent or legal guardian of the player, or the player is 18 years of age or older.
• You authorize On Deck Designs to use the submitted photos and information solely for the purpose of creating your custom order.
• You understand that your player's image may be stored securely for the purpose of future reprints, replacement orders, or seasonal drops — and that you may request deletion of this data at any time by contacting us at ondeckdesigns@proton.me.
• You confirm that the photos submitted are owned by you or that you have the right to use them for this purpose.
